Just a Quick note,

If Power Apps forms not able to show the fields or causing any error from the SharePoint Connection, it might be possible that some one have update the fields. And as per connection, it save schema of the List at the time of SharePoint connection creation.

To resolve this Go to the Data Source and refresh the Connection as per the below image
